Post-Hospital & Post-Operative Nursing
We support recovery after a hospital stay or surgery — monitoring your progress, managing wounds and medications, watching for complications, and helping you regain confidence so you can heal safely at home.
Wound Care Management
Assessment, dressing and ongoing management of acute and chronic wounds. Our nurses provide evidence-based wound care — including surgical wounds, pressure injuries, leg ulcers and complex or slow-healing wounds — with regular review to support healing.
Medication Reconciliation, Review & Monitoring
Administration, oversight and education for safe medication use. We review your medications, support correct administration, monitor for side effects and interactions, and help you and your family understand and manage your medication routine with confidence.
Complex Clinical Nursing Plans
Specialised nursing for complex health needs — including indwelling catheter (IDC) care, stoma care, nasogastric (NGT) and enteral feeding, diabetes management and continence support. We develop and deliver tailored clinical plans for ongoing conditions.
Care Coordination Support
Linking you with the right services for a joined-up, multidisciplinary approach. We help coordinate your care across GPs, specialists, allied health and support providers — so everyone is working together and nothing falls through the gaps.

How your care is funded
Care is delivered through NDIS, Support at Home, TAC, private health insurance and self-funded arrangements. DVA nursing services coming soon.
